KHA Quality Webinar: Frailty: A Unifying Concept for Care of the Older Adult
On November 7, 2024, the KHA quality department presented the KHA Quality Webinar: Frailty: A Unifying Concept for Care of the Older Adult. The Age Friendly Structural Measure was added to the Centers for Medicare and Medicaid (CMS) Final Rule on August 8, 2024, and started with the CY2025 Reporting Period, affecting FY2027 payments. Domain 3 of the CMS measure focuses on Frailty Screening & Intervention, requiring hospitals to screen for cognitive impairment/delirium, physical function/mobility, and malnutrition for early detection and intervention. Guided by the Institute of Healthcare Improvement (IHI) Age Friendly 4Ms (What Matters, Medication, Mentation, Mobility), Dr. Bill Bryant assisted in addressing key components of this measure and the interconnectedness of these elements. Frail patients are at high risk for geriatric syndromes like falls, delirium, and poor outcomes, making a holistic approach essential for optimal care.
