Community Coalitions

KHA is partnering with hospitals and community partners to provide assistance to members’ communities for the purpose of improving local health care and collaboration. The partnership aims to support local community efforts to improve a local and mutually agreed upon goal such as chronic disease management, care transitions, patient safety or behavioral health and social drivers of health. These communities provide a collaborative model to address community-specific issues with a broad range of community partners.

What is a Community Coalition?

A coalition is an alliance of individuals and/or organizations working together to achieve a common purpose. Each coalition has a common goal which helps to establish and build relationships within a community where better ideas and outcomes can be achieved together.

You are encouraged to participate in a Community Coalition if you are a:

  • Patients and/or Families
  • Clinicians practices (Physician, nurses, etc.)
  • Nursing Homes
  • Hospitals
  • Home Health
  • Hospice Agencies
  • Pharmacies
  • Area Agencies on Aging (AAA)
  • Community Health Centers
  • Fire Departments or EMS Providers
  • Patient Advocacy Organizations
  • Research Institutions and Universities
  • State and Local Agencies
  • Medical/Professional Associations (AMA, AHA, APA, etc.)
  • Faith Based Organizations
  • Community Organizations Active in Disasters
  • Senior Housing
  • Local AARP Chapters
  • Substance Abuse Recovery Organizations
